Militia quest not unlocking despite meeting requirements

The last subquest of the main quest line 'Militia' (also called 'Militia') is still marked as locked in my questbook, despite long ago having met the listed requirements for unlocking.

Does anyone know if there is any unlisted requirement to fulfil for it to unlock, other than getting to lvl21 and having finished the previous subquest?

Or any other instruction on how to get it to unlock.

Militia has a max level of 21, which means if your 22 before you get it, you never will.
